# EXPLANATION TEXT FOR THE PROCESSING OF PERSONAL DATA
**Osmangazi Private Oral and Dental Health Services Trade Limited Company** shall be hereinafter referred to as the “COMPANY,” and the “Law on the Protection of Personal Data” shall be referred to as KVKK.
This explanation text is prepared within the framework of Article 10 of KVKK, titled “Obligation of Data Controller to Inform.” During the collection of your personal data, it is prepared in order to inform you in the most transparent manner about the purposes for which your data shall be processed, to whom and for what purposes your processed data may be transferred, the method of collecting your personal data, the legal reasons, and your rights specified in Article 11 of the relevant law when we fulfill our obligation to inform as the COMPANY.
As the data controller, the COMPANY shall collect, process, store, update in case of any change in data, and transfer your personal data to third parties under the conditions specified in the law, provided that it does not violate the legislation. The explanations provided for your “Personal Data” in this Explanation Text also cover your “Sensitive Personal Data.”
## PERSONAL DATA COLLECTED FROM YOU AS THE DATA SUBJECT
### For Patients Who Are to Be Examined or Treated:
– **Your Identity Information:**
– If you are a Turkish citizen: Name, surname, Republic of Turkey ID number, gender, date of birth, identity card number.
– If you are not a Turkish citizen: Name, surname, nationality, temporary identity number or passport number, gender, date of birth.
– **Your Contact Information:**
– Your address in Turkey.
– Your GSM number and email address.
(YOUR DATA IN ARTICLES 1-2 ABOVE ARE PROCESSED IN CASE YOU SHARE EXPRESS CONSENT ACCORDING TO ARTICLE 5/1 OF KVKK.)
– **Your Test Results Obtained During Examination and Treatment:**
– Laboratory and imaging results, dental x-rays, test results, examination data, prescription details, previous illnesses, permanent diseases, blood type, and other health data obtained during medical diagnosis, treatment, and care services. These are processed to monitor the process in your patient tracing file created within the COMPANY during your examination and treatment (KVKK 6/3f).
– **Your Financial Data:**
– Payment and invoicing details.
– Private health insurance and Social Security Institution (SSI) data. These are processed for the planning and financing of health services.

– **Your Comments and Evaluations:**
– Responses and evaluations on the COMPANY’s website and social media accounts. (THESE DATA ARE PROCESSED AS THEY ARE PUBLICIZED BY THE DATA SUBJECT AS SPECIFIED IN KVKK 6/D.)
– **Closed-Circuit Camera Records:**
– Video recordings taken during your visits for examination and treatment within the COMPANY. (Explanatory texts are available under each camera for your easy access.)
### For Visitors:
– **Your Degree of Relation to the Patient:**
– Closed-circuit camera and voice recordings taken during your visits to the COMPANY. (QR codes under each camera lead to an explanatory text.)
### For Personnel and Doctors:
– **Data Collected During Job Application:**
– Name, surname, gender, educational background, specialty field information, GSM number, email address, foreign language proficiency.

## PURPOSES OF PROCESSING PERSONAL DATA
– Processed with express consent within the scope of KVKK Article 5/1.
– Processed to fulfill legal obligations under KVKK Article 5/1ç.
– Processed if data is publicized by the data subject within the scope of KVKK Article 5/1d.
– To evaluate job applications and check the compliance of experience, knowledge, and skills with the sought criteria.
– To fulfill obligations arising from employment contracts and/or legislation.
– To ensure the security of COMPANY fixtures and resources.
– To execute contracts signed between the parties and control the authority of signing parties.
– To plan commercial and business strategies in line with COMPANY procedures and legislation.
– To ensure the accuracy and actuality of data collected from patients, personnel, doctors, and business partners.
– To plan and execute skill and career development activities for COMPANY personnel and doctors.
– To create visitor and patient records and ensure discipline, security, and checks within the COMPANY.
– To inform authorized public institutions and organizations when required by public safety.
– To conduct necessary operational activities within the COMPANY, manage human resources, and analyze data for the improvement of health services.

– Ministry of Health and affiliated institutions.
– Private insurance companies.
– Social Security Institution (SSI).
– Law enforcement and judicial authorities.
– Laboratories, medical centers, and health service providers with whom the COMPANY cooperates.
– Legal representatives authorized by you.
– Attorneys, tax consultants, and auditors from whom the COMPANY receives consultancy.
– Regulatory and supervisory institutions and official bodies.
– Suppliers and cloud service providers with servers located within Turkey. (DATA IS NOT TRANSFERRED ABROAD.)
## COLLECTION METHODS AND LEGAL CAUSES OF PERSONAL DATA
Your personal data may be collected automatically or manually in electronic or physical environments. Legal causes include:
– Private Hospitals Regulation.
– Regulation on the Processing of Personal Health Data and the Protection of Privacy.
– Health Services Fundamental Law No. 3359.
– Other relevant laws and regulations.

Under Article 11 of KVKK, you have the right:

– To request deletion or destruction of your data under Article 7 of KVKK.
– To request notification of corrections or deletions to third parties.
– To object to results against you arising from automated processing.
– To claim compensation for damages arising from unlawful processing.
